• Confirm which specific facility category applies to your treatment. Several hospitals are empanelled only as “Exclusive Eye,” “Exclusive Dental,” or “Lab” — meaning they’re not empanelled for general multispeciality treatment even if the same group operates other hospitals that are.
• Effective dates vary hospital to hospital, ranging from April 2026 through late July 2026 in this list — don’t assume every entry has been active since the same date.
